What is a Geriatric Care Manager (GCM)?

A Geriatric Care Manager (GCM) is a healthcare professional who helps families care for aging loved ones, while encouraging as much independence as possible. As such, their function is more comprehensive than a mere caregiver. They communicate with your loved one’s physicians and medical caregivers; create a safe living environment; hire and monitor personal caregivers; manage hospice and/or palliative care; and perform a range of other duties.

Further, quality GCMs usually have experience and credentials in various areas. For instance, according to National Association of Professional Geriatric Care Managers estimates, of their 2,000 members, 35% have nursing degrees, 50% have social work or counseling backgrounds and 15% have advanced degrees or experience in areas such as physical therapy. So, if a senior was faced with the challenges of rehabilitation, for example, a GCM with an expertise in physical therapy might be the right choice. If ongoing medical issues are the primary concern, a GCM with a nursing background could be more desirable. Or, if difficult behavioral issues related to dementia, Alzheimer’s disease or other mental or emotional conditions exist, a GCM with a degree in psychology may be most appropriate. For this reason, the Senior Solutions Team is comprised of a variety of GCMs with a combined total of 75 years experience and credentials and degrees in multiple senior care fields.

Our client families and their aging loved ones determine the level of involvement of our GCM in the management of their care needs. We partner with local families, as well as those afar.

What we do as Caregivers

Our team understands that caring for an aging loved one, especially one with chronic or recurring health issues, is not easy. It’s a time-consuming, stressful responsibility that places pressure on the entire family.

We partner with the senior and their family to address all areas of need in order to reduce stress on the entire support system. Every client and caregiver is monitored by one of our licensed Geriatric Care Managers.

Our carefully selected, well-trained caregivers are the eyes and ears of the GCMs. This assures that changes in the client’s condition are addressed early and that the client is continually receiving the appropriate level of care.
